Patient Forms

If you are a new patient to our office, please visit our Patient Forms page to download our new patient bundle with forms that will need to be filled out when you arrive at our office. Printing them, filling them out and bringing them with you will allow us to attend to your medical needs more quickly than completing them on your arrival. Payment Options

Payment is due when services are rendered. We accept, cash, check, credit card, and in-house financing such as Care Credit.

Dental Insurance

We accept and honor most insurance plans and we file claims on your behalf, saving you the time and hassle. The following are just a few of the dental insurance carriers we're preferred providers for: Ameritis, Cigna, delta dental . GEHA Connection Dental, Humana and Medicaid
